Our investigation aimed to identify any risk factors correlated with 12-month recurrence and non-radical removal of non-melanoma skin cancer tumors subjected to intraoperative frozen-section (FS) histopathological assessment; and to evaluate if FS histopathological evaluation should be recommended for certain patient groups.
A retrospective cohort study, conducted at a single center, analyzed patient charts from 2017 to 2019 for those primarily treated with FS-aided excision. To explore risk factors for non-radical excision, a multiple logistic regression model was chosen as the analytical tool.
A total of 655 patients were involved in the investigation; 521 individuals were diagnosed with basal cell carcinoma (BCC), and 134 were diagnosed with squamous cell carcinoma (SCC). When compared to nodular basal cell carcinoma (BCC), radical excisions were less frequent in the initial surgical treatment for superficial, morpheaform, and infiltrative BCC subtypes, notably for infiltrative BCC with an odds ratio (OR) of 0.48 (95% confidence interval (CI) 0.29-0.77), demonstrating statistical significance (p < 0.001). Our study concludes that patients with superficial, morpheaform, and especially those with infiltrative BCC types might require FS, contrasting with the needs of patients presenting with nodular BCC. The COVID-19 pandemic significantly worsened the risk of food insecurity for older adults, simultaneously compelling adjustments in how government entities and social service organizations could assist this population, especially given their disproportionate vulnerability to the virus. The study’s goal was to understand the perspectives of social service providers on how low-income, community-dwelling older adults’ access to food and related resources transformed during the COVID-19 pandemic. Data were gathered through in-depth interviews with 22 social service providers working for public and private social service agencies located in Oregon. Responses revealed that the pandemic’s impact on older adults’ food access was influenced by amplified public benefits and the need for social distancing. Participants’ observations suggest that temporary SNAP allotments aided in the food security of older adults. Subsequently, social distancing regulations disrupted the established patterns of food procurement, including visits to grocery stores, receiving assistance from family or neighbors, getting supplies through social programs, and attending communal dining venues. In order to reduce reliance on in-person transactions and maximize technological use, food assistance programs modified their procedures. When researching the food security of senior citizens, future investigations should address the practical hurdles and subjective experiences related to SNAP participation, the social support systems available to them, and the extent of their technological access and understanding.
A growing atherosclerotic plaque carries a heightened chance of a stroke. Our investigation was designed to analyze the association between various stroke risk factors and modifications in carotid plaque thickness, as visualized via ultrasound procedures.
A follow-up examination of the carotid bifurcation plaque study’s data was carried out by us. Sonography data were gathered from the sonography labs within two university hospitals. 564 patients (240 of whom being male, and comprising 426% of the men), with atherosclerotic plaques in the carotid bifurcation and internal carotid artery, and a 30% stenosis visible by duplex sonography were part of the study. An extreme gradient boosting algorithm, coupled with the Shapley additive explanation method, enabled the development of machine learning models to determine critical risk factors and their interactions. Thirty-six months later, the carotid plaque thickness experienced a shift, with initial plaque thickness and stroke risk factors as influential determinants.
Regression models were individually developed for both the left and right carotid arteries. The left model exhibited an R-squared value of 0.964, while the right model’s R-squared value was 0.993.
